The Hospice Liaison Admission Nurse coordinates and manages the hospice referral and admission process. Develops and implements a hospice plan of care in consultation with Hospice medical director that meets each patients specific needs and is in compliance with JCAHO, federal and state regulations, reimbursement guidelines as well as agency policies and procedures. Initiates discharge planning at time of evaluation and /or admission to inpatient hospice. Make daily visits on inpatient hospice patients and is responsible for the professional management of the patients care in accordance with the hospice plan of care. Works collaboratively with appropriate hospital personnel and communicates plan of care. Communicates effectively with all interdisciplinary team members, referral sources, and all divisions/programs in the agency. Provides nursing care, emotional and spiritual support to terminally ill patients, families, caregivers, and facility personnel. Hospice Liaison provides hospice services, benefit and philosophy to patients and families, and evaluates patient for hospice appropriateness in the inpatient setting. Assists case managers with routine visits, symptoms management visits and death calls as to hospitalized patients. Assists with field Hospice evaluations and admissions as needed. Performs other duties as assigned. Maintains ANA standards of professional nursing practice. Assesses, analyzes, validates and integrates data including labs to assess patients physical, psychosocial, safety, learning, spiritual, cultural, age-specific, special risk and self-care needs, problems and variances, and eligibility for hospice. Performs patient visits to assess patients eligibility for hospice. Overseas the inpatient care of a hospice patient. Appropriately evaluates new patients for criteria for hospice general inpatient level of care and current inpatients for criteria for continued stay in the hospice general inpatient level of care. Educates patient, family and caregivers on signs and symptoms of approaching death and end of life care. Involves patient/family in the formation of the plan of care. Develops, implements and updates an individualized plan of care based on identified patient care needs. Includes nursing, teaching, continuum of care and discharge planning. Meets with patient/caregiver once referral for hospice services received. Effectively communicates information on agency services. Identifies learning needs, abilities, preferences and readiness, support systems, coping mechanisms and barriers to learning. Advises the hospice team of additional services needed. Assesses current medication regimen for patient and makes recommendations regarding dosing criteria from Hospice formulary. Possesses knowledge of appropriate management and care for Hospice patients with infusion needs and/or drainage devices.  EOE Minorities/Women/Disabled/Veterans. VEVRAA Federal Contractor.

Required: Illinois RN License. 5 years Hospice RN experience. Drivers License. BLS. Basic computer knowledge. Ability to communicate effectively in English, both verbally and in writing. Excellent customer service skills. Consistently works within the framework of the Illinois. Practice Act for Nursing and ANA Standards of professional nursing practice. Ability to assess and determine situations requiring an interpreter and to use interpreter services effectively. Desired: BSN.
